About TruEvolution
TruEvolution is a community-rooted power-building and direct service organization based in California’s Inland Empire. We transform lives by meeting urgent needs today—while building the long-term community power needed to create healthier, safer, and more equitable conditions for everyone.
We know health is shaped by far more than a doctor’s visit. It’s shaped by whether people have stable housing, economic opportunity, mental health and wellness support, affirming spaces, and access to culturally responsive care—and whether communities have the power to influence the systems and policies that govern their lives.
That’s why TruEvolution operates at the intersection of power-building and direct services. Our integrated model connects the support people need across the full spectrum of well-being, including public health programs, transition housing, housing supportive services, mental health and wellness, and workforce development—alongside the TruHealth & Wellness Center, our primary care and specialty medical clinic.
We are building a regional Center of Excellence focused on addressing the social determinants of health—housing, access to care, economic stability, community connection, and civic participation. By aligning advocacy, services, and community infrastructure, we strengthen individual outcomes while advancing broader systems change.
Our Founding
TruEvolution was founded in 2007 as a youth advocacy organization dedicated to uplifting marginalized voices and advancing equity in the Inland Empire. In its earliest years, the organization focused on youth leadership development, civic engagement, and confronting inequities impacting communities disproportionately affected by HIV and other health disparities.
Grounded in community organizing and advocacy, TruEvolution built its identity around dignity, culturally responsive support, and community-led solutions. As the organization listened to residents and leaders across the region, one truth became clear: policy advocacy alone was not enough. Communities also needed accessible, affirming, and coordinated services that addressed urgent needs in real time.
Over the past decade, TruEvolution expanded into a comprehensive direct service provider while maintaining its power-building foundation. Today, we operate at the intersection of advocacy, community infrastructure, philanthropy, and integrated care—connecting immediate services to long-term systems transformation.
In 2023, we launched Project Legacy, establishing a permanent campus that anchors our growing continuum of housing, health, and community programs with an average waitlist of 500+ a year. In 2024, we opened the Inland Empire LGBTQ+ Center, expanding access to affirming space, programming, leadership development, and civic engagement opportunities.
TruEvolution also plays a growing philanthropic role in the region. Through community giving and sponsorship initiatives, we reinvest resources locally. As a fiscal sponsor for more than ten organizations—stewarding over $1 million in funding—we enable and strengthen grassroots community-based organizations and start-ups working to advance equity across the Inland Empire.
From youth advocacy in 2007 to a regional anchor institution today, TruEvolution continues to evolve—grounded in the belief that transforming lives requires both direct support and sustained community power.
The Inland Empire
The Inland Empire spans more than 27,000 square miles, making it larger than five U.S. states. The region stretches across urban, suburban, rural, and desert landscapes—reaching to the borders of Nevada, Arizona, and Mexico. It is one of California’s fastest-growing and most diverse regions, yet it has historically received fewer investments in public health infrastructure, culturally responsive care, and coordinated community-based systems than other parts of the state.
Its size and geography create unique challenges. Residents often travel long distances to access health care, stable housing, or affirming services. Transportation gaps, workforce shortages, income inequality, stigma, and fragmented systems of care compound barriers—particularly for marginalized communities. Rapid population growth has outpaced infrastructure in many areas, leaving families navigating disconnected systems without integrated support.
TruEvolution exists in the Inland Empire because this region deserves investment, permanence, and power. We are building a community ecosystem where people can access primary care and specialty services, stabilize through housing and supportive programs, receive mental health and wellness support, and pursue economic opportunity—all while engaging in leadership development and advocacy.
Through initiatives like Project Legacy, our campus launched in 2023, and the Inland Empire LGBTQ+ Center, launched in 2024, we are investing in permanent community infrastructure—physical spaces that foster belonging, healing, leadership development, and collective action across this expansive region.
Our commitment is long-term. We are not only delivering services across 27,000 square miles—we are building institutions that ensure Inland Empire communities have the resources, coordination, and collective power needed to thrive.
