I looked at the URL you’ve shared, and on my end I’m able to select multiple image options. Here’s a screenshot.
Maybe you’ve found the options or if the issue on your end is still there when you have some time, can you please share the steps I can follow to replicate the issue?

Hi and thanks for having a look at my problem. I found my problem (put it down to user error), I had selected Multiple Choice instead of Checkboxes. Once I deleted the multiple choice option and updated the form using Checkboxes, everything worked well.
WPForm is a 1st class plugin which is easy to use. The only comment I would say is that I did look at the documentation and could not find a description which shows the difference between Multiple Choice instead and Checkboxes. That said I’m very happy with WPForm.
