    Hello Forminator Support Team,

    I am experiencing an issue with the Forminator plugin on my WordPress website. Some dropdowns are not displaying correctly, and certain tick marks appear missing or blurry. Additionally, file uploads do not function as expected.When the Issue Started:

    • The issue began after installing the LiteSpeed Cache plugin.
    • When I deactivate and reactivate the LiteSpeed Cache plugin, Forminator works fine for a short period (around a day), but then the issue returns.

    Troubleshooting Steps Taken:

    1. Cleared Cache: Tried clearing the LiteSpeed Cache and browser cache.
    2. Excluded Forminator Scripts: Added Forminator-related scripts and styles to the LiteSpeed Cache exclude list.
    3. Adjusted Optimization Settings: Tweaked JavaScript/CSS minification and defer settings in LiteSpeed Cache.
    4. Plugin Deactivation: Deactivated and reactivated both LiteSpeed Cache and Forminator, but the issue persists.
    5. Checked Theme & Other Plugins: Switched themes and disabled other plugins to see if there were conflicts.

    Temporary Fix:

    • If I deactivate and reactivate the LiteSpeed Cache plugin, Forminator works fine for some time but stops again after a while.

    Request for Support:

    • Is there a specific setting in LiteSpeed Cache that is causing this conflict?
    • Are there known compatibility issues between Forminator and LiteSpeed Cache?
    • Any recommendations for a permanent fix?

    Hope you’re doing well today! Sorry to hear about the issue you’re facing with the Forminator Plugin.

    Thank you for the detailed information about the issue. I tried to replicate the issue on my lab site with Forminator form and LiteSpeed Cache plugin – however, I was not able to replicate the issue on my lab site.

    Additionally, I also checked the page you’ve shared and the form looks fine the dropdowns, date pickers and upload field worked fine for me. However, it could be possible that you have the LiteSpeed Cache Plugin disabled at the moment.

    In this case, can you please enable “Load form using AJAX” and “Prevent page caching on form pages” for this form and see if that helps?

    You will find these options under WP Admin >> Forminator >> Edit form >> Behavior >> Rendering

    In case this doesn’t help, can you please confirm if you are using the latest version of the Forminator plugin? Also, could you please share the Forminator form export with us so that we can replicate if the issue occurs on that specific form?

    To share the form export
    – Navigate to the “Forminator -> Forms” page in your site’s back-end
    – click on a little “gear” icon next to the form in question
    – select “Export” option
    – copy given export code and paste it at https://pastebin.com
    – include link to that paste in your response below.

    Note: this export will not include any data that is already submitted – they’ll stay safely on your site and we don’t need them anyway.

    I tried enabling “Load form using AJAX” and “Prevent page caching on form pages”, but unfortunately, the issue still persists.

    Thank you for the form.

    I couldn’t replicate the issue on my lab site, I see the form on your site is also the same as it shows in my site when I imported it.

    Could you try by excluding the Forminator JS files from the front end optimization in case you are using it?
