Is this a “theoretical” question in regards to that 503 Service Unavailable thread you have as well? In cases where maybe a person signed up during that maintenance window and thus didn’t get processed all the way through?
I know we have some intended safeguards for those moments where we try to email the notification about the signup, in cases where “notify by email” is disabled, because we don’t want the site owner to miss out completely on a contact. At least in those moments there would be the email notification that could be used to manually fill in a contact after the fact.
If that’s not quite what you’re referring to, we’re all ears for some clarification so that we can help as best we can.
No, the missing notification is not related to the other topic. I tested this today. We have notifications turned off. Which e-mail address would receive the warning? The Constant Contact owner address? A website address?

If you’ve set a custom email address to receive notifications in the form’s settings, I believe it should be that one, otherwise it’d fall back to the admin email address set in your General settings.
I double checked, neither the notification e-mail address nor the WordPress admin received a notification about failed signups. My concern is that the end user did not see a warning either.
Just to be certain, were spam filters checked as well?
