Never mind. The form was in draft mode, not published. Now it works! Great plugin!
Back to the same problem. Suddenly, the Forminator Pro form is no longer displaying. Tried recopying the shortcode, creating a new form, etc. This is the page: https://visionfieldtechnologies.com/

Hi Harish, My apologies. We had unpublished the pop-up because the site is live and the client didn’t want it up. It is now up. Also, the site was built on a temporary host and just went live; perhaps that has something to do with it? If you need access to the back end, let me know. Thanks!

To test a popup you could set it to only show on a specific test page.
If there is any issue with content inside a popup we first need to make sure the issues is related to Boxzilla.
Please create a page under Pages > Add new, and put the shortcode that you are having trouble with there on the page (without Boxzilla box) and see if it works normally there. If it does, please share a link to that page and to another page where you have the Boxzilla box with that same shortcode in it so that we can compare those.

Under Forminator > Form > edit > Settings, is it set to Load form with Ajax? Please turn that off.
