Reporting

The CalAIM Justice Involved reentry program requires the development of reports to track the health utilization, pre-release services, and the warm linkages and handoffs into community-based systems of care. Serrano has leveraged AI solutions to automate the reentry care plans, to simplify the reporting requirements to the DHCS, to improve eligibility screenings, and to maximize revenues for pre-release services.  The technology platforms are designed using Microsoft’s Power BI solutions and integrates into the production environments for the Sheriff’s Office, Probation Department, and Behavioral Health Agency.

Justice involved reporting features include the following:

  • Medi-Cal eligibility screenings and new Medi-Cal applications
  • Mental health screenings and assessments
  • Substance use screenings and assessments
  • Average length of stay & population for CalAIM eligible populations
  • Prevalent health conditions, chronic disease and conditions
  • Mild, moderate, and persistent mental illness
  • Offsite medical appointments for specialty care
  • Linkages and warm handoffs to post-release Enhanced Care Management providers and Medi-Cal health plans
  • Linkages and warm handoffs to post-release County Behavioral Health for specialty mental health and addiction medicine services
  • Linked referrals directly to community-based providers
  • Linkages to services for individuals being released into a different county
  • Short stays and quick releases from the detention facility
