This form is specifically for auxiliary Leaders that need to request funds for their auxiliaries solely requesting funds to purchase equipment, gifts, supplies needed for auxiliary etc. Please utilize Event Check List if you are planning an event. You do not need to fill out both forms if you are planning an event, *ONLY* the event check list where funds requests are listed. Please do not duplicate. Auxiliaries Included but not limited to - Media - Children's Church - Shepherds Care - Impactor Care - Outreach - Arts *** ADVANCE NOTICE REQUIRED *** Without proper advance notice, your request can be denied. The Finance team strives to meet all needs of the ministry, but without advance notice, we may not be able to accommodate your request. Please see below for the submission schedule based upon dollar amounts for proper budgeting. 2 Weeks Prior - Under $500 One Month in Advance - $501-1000 Two Months in Advance - $1000+

Outreach Event Planner

This form must be submitted 2-3 months prior planning event. Please ensure to connect with all ministry leaders (not volunteers) to solidify ministry planning and involvement. If this form is not minimum of 2 months prior to event, please select date that is comparable to the 2 month mark, or the form will be rejected back to ministry leader for requesting new dates. Flyer requests for all events will be handled by administrative staff upon approval. INSTRUCTIONS: Please adhere to the church calendar for scheduling events - if this event is NOT on the calendar, please seek approval from your Director. If MMCC Facility is needed - please ensure you have confirmed facility availability with the Director of Facilities, Deacon Donald Banks.
