CrossRoads transitional housing is a 60-to-90-day structured program paired with outpatient services. All clients admitted into our transitional housing are enrolled in our outpatient clinical services. Although our housing program has autonomy in its structure we remain clinically driven and work very closely with our outpatient clinical team. We recognize that a structured environment paired with treatment services helps the clients have the best success at long
term recovery. Our housing team provides a culture of belonging, acceptance, and respect.

Comprehensive Case Management & Life Skills Support

Every client in our transitional housing program is paired with a dedicated case manager who helps them secure essential identification documents, transportation resources, and medical services including psychiatric care, primary care, dental, vision, and medication-assisted treatment when appropriate. Clients also receive support with education and employment, including GED assistance, vocational training, job readiness, and job placement. Our team works closely with community partners to provide financial resources, life-skills development, and support navigating legal responsibilities — ensuring every client has the tools and guidance needed to move toward independent living.
