Step 1: Fill Out the NDIS Referral Form
Fill out our form via our website or call us to discuss your needs.
Step 2: Sign and Return Your Service Agreement
We will send you an individualised service agreement to sign to ensure we are on the same page.
Step 3: Detailed Assessment
Your allocated therapist will complete a thorough assessment with you to tailor services to your specific requirements.
Step 4: Treatment Plan
We’ll create a personalised service plan that aligns with the participant’s goals, funding and preferences.
Step 5: Start Intervention
Book your first intervention appointment with your therapist to occur wherever you feel most comfortable.
Step 6: Achieve your goals!
Your treatment plan will be an evidence-based intervention to help you work towards your goals, one session at a time.

What is an NDIS Physiotherapist?
A functional movement expert, who works with people with disability!
An NDIS physiotherapist is a qualified, registered allied health professional. Generally, physios help you manage physical difficulties resulting from injury, ageing or illness to improve your movement and physical function. Our NDIS physios have expertise to help you manage difficulties arising from your disability.
Our mobile NDIS physios will complete an assessment and provide recommendations based on your disability, goals and current function or fitness whilst also considering your medical history. We’ll then provide targeted therapy, assistive technology and/or an exercise program, and teach you or your support people to complete it safely and effectively.

How can I access services?
Anyone can access our services, you don’t need an NDIS plan. You can contact us to discuss your needs and set up an initial consultation. We will guide you through the process.
Do you offer home visits?
Yes, we provide mobile physiotherapy services – with free travel for physiotherapy services. Our therapists can visit you at home to ensure you receive the care you need. This service is especially beneficial for those with mobility challenges or people who would prefer to avoid a crowd and loud noises typical of a clinic space. It also offers the benefit of implementing an intervention in the space it’s most needed!
Are Active Ability’s Physio’s services mobile?
Yes, we offer mobile physiotherapy services across Sydney and the Central Coast – with free travel! Our team comes to your location for convenience; this ensures you receive care in a comfortable environment. Being mobile does not mean we’re restricted to only meeting you at home though, we meet our clients at:

- A local pool or hydrotherapy facility
